Patient safety is increased when we reduce unwanted variability in care. We always wash our hands, never forget to check for allergies and observe all the steps in the checklists. Every test is followed up and none fall through the gaps. Never ever events just don’t happen. Yet we are only human. Most of the time we are careful and diligent but, even when the stakes are higher than usual in the middle of a pandemic, sometimes care and diligence isn’t enough.
